San Francisco Healthy Homes Community Action Plan

The City and County of San Francisco, Department of the Environment (SF Environment), in collaboration with Department of Public Health (DPH) and other governmental agencies, health service providers, community organizations and local residents conducted a project, called San Francisco Healthy Homes (SFHH), to address health inequities in the Southeast area of San Francisco. This Community Action Plan (CAP) reflects the work of SFHH’s stakeholders who met and conducted a strategic planning process over a two-year period. The CAP will help guide and transform the community’s vision of healthy homes and neighborhoods into achievable goals and actions.
